Annual Shooting Sports County Reporting Forms
County 4-H Shooting Sports Programs are required to report their activities for October 1 to September 30 of the current year. Please use the “County Summary” form to report any 4-H Shooting Sports activity from certified instructors in your county. The “Club/Local Level Instructor Report” form may be used to retrieve information from instructors. Include an inventory list of Shooting Sports equipment of significant value in your annual report.
The “County Summary” is due to Scott Reese in the State 4-H Office by October 15. A set of instructions is provided as a guide to completing these forms. Contact your County Extension Office for more information.
