Tennessee 4-H Foundation

Many of you are in the process of submitting scholarship requests for your 4-H members who have won college scholarships for their outstanding 4-H work. The scholarship request requires evidence the student has successfully completed one quarter/semester of post high school work and be enrolled for the next unit of schoolwork. We verify the member has met these requirements by having them submit a transcript of their grades with the request form.

Since grades are considered sensitive information and protected by FERPA (Family Educational Rights and Privacy Act), this information MUST be sent to us through a secure system. When sending the scholarship payment request form with the transcript by email, please use UT Vault. This is a se- cure system that will encrypt, or disguise, the content of email message in order to protect sensitive information from being read by anyone other than intended recipients. You may go to UT VAULT – SECURE COURIER for information on how to use this system. Send the F837: 4-H Scholarship Payment Request along with the transcript to tn4h@utk.edu. We will quickly process the request and get it to the student’s educational institution.
